Clinical Review of Diverticular Disease of Cecum and Ascending Colon


Abstract
Diverticular disease in Oriental populations is located predominantly to the right side colon, in contrast to the left sided predilection observed in Western population. We experienced 19 cases of diverticular disease of the right side colon from
Jaunary 1988 to August 1991 at the Department of Surgery, Chung Goo Sung Sim Hospital. Of 19 cases, 7 cases were under 40 years of age, whereas 12 cases were over 40 years of age. There were 12 men and 7 women. Of 19 cases, 11 cases were found in
the
cecum alone, 7 cases in the cecum with ascending colon, one case in the ascending colon alone. Of 19 cases, 10 cases were managed by operative treatment and 9 cases were managed by conservative treatment. the surgical procedure of the colon
diverticulitis were appendectomy only(5 cases), appendectomy with diverticulectomy (2 cases), appendectomy with stump inversion(2 cases), right hemicolectomy(1 cases). Among 10 cases of surgically treated colonic diverticular diseases, the
commonest
preoperative diagnosis was acute appendicitis(8 cases). It is desirable that, if possible preoperatively, barium study is performed at the case of mimicking appendicitis. Because cecal diverticulitis is difficult to differentiate from acute
appendicitis
on the basis of the clinical finding. Cecal diverticulitis is found at the time of laparotomy due to appendicitis, if possible, diverticulectomy appears to be appropriate treatment.
